Request to Create an Existing Course as Individual Instruction

Faculty may use this form to request registration for a student who is receiving the same content and learning outcomes of a course that exists in the catalog which may not be offered in the term or at a time the student can take it.

The faculty member and student should have a conversation prior to this form being submitted to discuss why this class needs to happen outside of an officially scheduled section and determine the meeting structure. Requests should be submitted before the last day to add for a term, please note the deadlines on the Academic Calendar.

Requests submitted by a faculty member will be processed by the Registrar's Office and a copy of the request sent to the student's provided email address.

If a student is attempting to earn 3 or 4 credits in Jan Term, they must begin work prior to Jan Term in order to meet the credit hour requirement.
