New Students: Submitting Your Health Forms

Welcome to Drew University Health Services (“DUHS“)! We hope that your time here will be healthy, happy and intellectually stimulating. We encourage you to come see us if you have any health care needs or concerns. Please read the following information carefully regarding health requirements.

Starting February 2023, the COVID-19 vaccine mandate will no longer be in effect and proof of vaccination will no longer be required. We strongly recommend that everyone follows CDC guidelines, which still recommend keeping up with vaccination including boosters.  If you would still like to submit a copy of your COVID-19 vaccine information, please upload it to myhealth.

Students are able to complete most of the required health documents before arriving on campus. We urge you to allow yourself sufficient time to access your immunization record from your healthcare provider and for undergraduates to make arrangements to also have a physical examination.  Note that these requirements are for new students only. Returning students do not need to resubmit health forms to the Health Service. If you are a returning athlete, be sure to check with your coach or trainer about your requirements.

Documents must be received by the due date indicated below. A late fee of $100 will be assessed if health forms are not received within 2 weeks after the deadline (August 1 for the fall semester, February 1 for the spring semester). As a registered student you have confidential and secure online access to forms on your electronic health record through myhealth, a secure student health portal.  Although you can complete most information online through the portal, there are some documents that need to be printed, completed by your healthcare provider, and returned to DUHS by uploading them through your student health portal. Both uploaded and online portal forms must be received by the due dates listed below. Students will not be permitted to continue registration or reside in Drew housing until immunization and information requirements are met.  Your health forms will only be considered complete after BOTH the uploaded and online portal forms are received.  All documents  must be in English.

Students admitted 2 weeks (14 calendar days) or less before the appropriate due date, or admitted after the due date,  have 30 calendar days from date of enrollment to submit forms on time.

3 Easy Steps for Completing Your Health Forms:

1. Print Out Paper Copies of:

  1. Admission Physical Exam Form Required for all CLA and INTO students; optional for CUE, Graduate and Theology and Summer Only Students. For athletes: you can hand in the physical exam form to athletics once you upload it to your portal.  See information below for ATHLETES.
  2. Immunization Record: Have your healthcare provider complete the form, then upload it to your portal. PLEASE USE OUR FORM TO MAKE SURE YOU MEET ALL THE VACCINE REQUIREMENTS which are clearly outlined on the form.
  3. Meningitis Information for Students: Print out and read for important information about meningitis. This does not need to be uploaded. It is for your information.
  4. Meningococcal Questionnaire: Print, complete, and bring to your healthcare provider along with your immunization record for guidance as to which meningitis vaccine(s) may be required for you. This form does not need to be uploaded to your portal, it is meant to serve as a guide.

Bring printed forms to your healthcare provider for completion.

2. Online Forms to be Submitted Through myhealth:

Log into myhealth portal to access your forms. Once complete, hit “submit.” These will not be accepted in paper form.

  1. Authorization for Treatment (if over 18 on arrival day)
  2. Privacy Practices Acknowledgement (all students)
  3. Admission Health History (all students)
  4. Meningitis Questionnaire (all students)
  5. Tuberculosis Low Risk Questionnaire (all students)

     Other Forms (Only if Needed): Print out, complete, and upload through myhealth:

  1. Parent/Guardian Authorization for Treatment for Under 18 (if student is under 18 on arrival day)
  2. TB High Risk Assessment Questionnaire This form is only required if you responded “YES” to any questions on the Tuberculosis Low Risk Questionnaire in your health portal. The form needs to be completed by a physician, NP, or PA.
  3. Medical Exemption All students must submit the paper Immunization Record. If claiming exemption, please write “Exempt” in the immunizations. This form must be completed and signed by a physician, NP, or PA.
  4. Religious Exemption All students must submit the paper Immunization Record. If claiming exemption, please write “Exempt” in the immunizations.
  5. Immunotherapy Treatment Plan (Allergy Shots). This must be completed and signed by a physician, NP, or PA if you plan to have the Health Service administer your allergy shots.
  6. Prescription of Restricted Medications During the School Year (e.g., Adderall, Ritalin, Concerta and other restricted medications). This must be completed and signed by a physician, NP, or PA if you wish to have the Health Service prescribe these medications for you while you are on campus. It must be completed annually.
  7. Medication Storage Waiver This form is only required if the Health Service will be storing medication, such as medication requiring refrigeration including allergy serum, or under other special circumstances. This request must be pre-approved by the Health Service. Students who take prescription and/or over-the-counter medications are responsible for obtaining and storing their own medication.

3.  Submit Your Paper Forms:

      Upload through the Document Upload feature in the web portal myhealth.

Paper forms will NOT be accepted. Once received, your forms will be reviewed by our staff. Please check your Drew email frequently, as information regarding the status of your forms and any missing items will be communicated directly to you by email.

If you are an ATHLETE: The athletic department has their own health forms and requirements. Please do not submit our health forms and immunization record to athletics. Submit all forms as outlined above, and follow instructions from athletics about their forms. The Health Service is not responsible for sending forms to athletics, or vice versa.

We must receive these forms prior to your arrival on campus to assure that your registration and move-in goes smoothly.

Immunization Requirements

Guidelines are established based on New Jersey law, Center for Disease Control and Prevention (CDC) guidelines, Advisory Committee on Immunization Practices (ACIP) guidelines, American College Health Association (ACHA) guidelines, and the University policies.

  1. Measles vaccination: proof of two doses after age one and thirty days apart, or laboratory proof of immunity if born after 1956.
  2. Mumps vaccination: proof of one dose or laboratory immunity if born after 1956.
  3. Rubella vaccination: proof of one dose or laboratory proof of immunity if born after 1956.
  4. Hepatitis B vaccine: proof of three doses if enrolled in 12 or more credits.
  5. Meningococcal Meningitis vaccine type A,C,Y,W135: within the last five years; one dose on or after age 16. Required for all new students: age 18 and younger; age 19 and older and living in campus housing; and for certain medical conditions as outlined in the meningococcal questionnaire. MenB vaccine required for all new students with certain medical conditions as outlined in the meningococcal questionnaire.

Immunization Recommendations (Not Required)

  1. Varicella (Chicken pox)
  2. Human Papillomavirus (HPV)
  3. Influenza
  4. Hepatits A
  5. Pneumococcal (Pneumonia/for high risk groups)
  6. Tetanus vaccine/Tdap within the last 10 years

 

Important Information About Submitting Your Health Forms

Some students have reported that they are unable to schedule appointments for physicals until after the deadline. When this happens, the Health Service will grant an extension. However, all forms must be received as soon as possible and prior to arrival in order to secure housing and registration.

Your documents are not complete until the Health Service receives all forms. No substitutions for online forms will be accepted. Please be sure to submit your forms before the appropriate due date listed under New Student Documents (above).
