Caspersen Students

• Leave of Absence/Withdrawal 
  • If the above link does not bring you to the Leave of Absence/Withdrawal form, please follow these steps:
    • Step 1: Copy and paste this link into the address bar of the browser (preferably Chrome), and hit Enter:  https://selfservice.drew.edu/prod/bzskloaw.p_request
    • Step 2a: If you were already logged into TreeHouse, you will be redirected to the Leave of Absence/Withdrawal form. Complete the form and submit.
    • Step 2b: If you were not already logged into TreeHouse, log in now. You will be redirected to the TreeHouse home page. Copy and paste the link above into the address bar again, and you will be redirected to the Leave of Absence/Withdrawal form. Complete the form and submit.
• Incomplete Grade Request scroll down to the student forms section and click the “Request an Incomplete” link.

• Petition to the Faculty
  • Includes: late registration, incomplete requests, tutorial registration, waiver of requirement, concentration declaration, etc.
    • Declaration of concentration: select “General Petition to Faculty” as the type on the petition to the faculty form and indicate which concentration to add in the text box.
• Re-Entry Application (Form is housed on the Center for Academic Excellence webpage under “Forms”)
• Atlantic Health Medical Humanities Certificate Completion Form 

Theological Students

• Leave of Absence/Withdrawal

    • Step 2a: If you were already logged into TreeHouse, you will be redirected to the Leave of Absence/Withdrawal form. Complete the form and submit.
    • Step 2b: If you were not already logged into TreeHouse, log in now. You will be redirected to the TreeHouse home page. Copy and paste the link above into the address bar again, and you will be redirected to the Leave of Absence/Withdrawal form. Complete the form and submit.
• Drew Theological School Community Fellow Application
• Incomplete Grade Request scroll down the page and click the “form” link under the “Request an Incomplete” header.
• Petition to Academic Standing
  • Includes: tutorial registration, waiver of requirement, etc.
  • Re-Entry: select “General Petition to Faculty” as the type on the Petitions to the Academic Standing Committee form and indicate which term you are requesting to return.
  • Exemption from a Course Requirement: select “Request for waiver of required course” as the type on the Petitions to the Academic Standing Committee form.
• Cross-Registration Form – to be used by Drew University students studying at a partner institution or students from General Theological Seminary, Union Theological Seminary, and NY Theological Seminary wanting to take a course at Drew.
• MOU (Cross-Reg) Form – to be used by students from Lancaster Theological Seminary or Palmer Theological Seminary
  • Cross-Registration (MMOU) Completed Form Submission
