3 Easy Steps for Completing Your Health Forms
Step 1: Complete online forms in myhealth:
Log into myhealth portal to access your forms. Once complete, hit “submit.”
- Authorization for Treatment (if 18 or over on arrival day). If under 18 on arrival day: print, complete, and upload the Parent/Guardian Authorization for Treatment for Under 18 form.
- Privacy Practices Acknowledgement (all students)
- Admission Health History (all students)
- Meningitis Questionnaire (all students)
- Tuberculosis Low Risk Questionnaire (all students)
Step 2: Print Out Paper Copies and upload completed forms to your health portal:
- Physical Exam Form: Requiredfor all undergraduate CLA, INTO, andtransfer students only. For athletes: you can hand in the physical exam form to athletics once you upload it to your portal. See information below for ATHLETES.
- Immunization Record: Required for all students. Have your healthcare provider complete the form, then upload it to your portal. PLEASE USE OUR FORM TO MAKE SURE YOU MEET ALL THE VACCINE REQUIREMENTS which are clearly outlined on the form.
Bring printed forms to your healthcare provider for completion.
The following forms are for your information and do not need to be uploaded. Please read carefully:
- Meningitis Information for Students: Read for important information about meningitis.
- Meningococcal Questionnaire: Print, complete, and bring to your healthcare provider along with your immunization record for guidance as to which meningitis vaccine(s) may be required for you. This form does not need to be uploaded to your portal, it is meant to serve as a guide.
Step 3: Upload Your Paper Forms:
Paper forms will not be accepted! All completed forms must be submitted through the Document Upload feature in myhealth. Use the drop down menu to choose the appropriate category (for example, Immunizations; Physical Exam). Make sure all forms have appropriate signatures where indicated and include your name and birth date. All forms must be in English or they will not be accepted. Once received, your forms will be reviewed by our staff. Please allow several days for processing of forms, and check your Drew email frequently– information regarding the status of your forms and any missing items will be communicated directly to you by email.
If you are an ATHLETE: The athletic department has their own health forms and requirements. Please do not submit our health forms and immunization record to athletics. Submit all forms as outlined above, and follow instructions from athletics about their forms. The Health Service is not responsible for sending forms to athletics, or vice versa.
We must receive these forms prior to your arrival on campus to assure that your registration and move-in goes smoothly.

OTHER FORMS (ONLY IF NEEDED): PRINT, COMPLETE, AND UPLOAD THROUGH MYHEALTH:
- Parent/Guardian Authorization for Treatment for Under 18 (if student is under 18 on arrival day)
- TB High Risk Assessment Questionnaire This form is only required if you responded “YES” to any questions on the Tuberculosis Low Risk Questionnaire in your health portal. The form needs to be completed by a physician, NP, or PA.
- Medical Exemption All students must submit the paper Immunization Record. If claiming exemption, please write “Exempt” in the immunizations. This form must be completed and signed by a physician, NP, or PA.
- Religious Exemption All students must submit the paper Immunization Record. If claiming exemption, please write “Exempt” in the immunizations.
- Immunotherapy Treatment Plan (Allergy Shots). This must be completed and signed by a physician, NP, or PA if you plan to have the Health Service administer your allergy shots.
- Prescription of Restricted Medications During the School Year (e.g., Adderall, Ritalin, Concerta and other controlled medications). This must be completed and signed by a physician, NP, or PA if you wish to have the Health Service prescribe these medications for you while you are on campus. It must be completed annually.
IMMUNIZATION REQUIREMENTS:
Drew University is committed to providing a healthy environment for all students. In order to protect students from vaccine-preventable diseases and to comply with New Jersey law (NJAC 8:57-6.1 to 8:57-6.18) and university requirements, students must submit the necessary health forms to the Health Service by dates listed above. Guidelines are established based on New Jersey law, Center for Disease Control and Prevention (CDC) guidelines, Advisory Committee on Immunization Practices (ACIP) guidelines, American College Health Association (ACHA) guidelines, and University policies.
- Measles vaccination: proof of two doses after age one and thirty days apart, or laboratory proof of immunity if born after 1956.
- Mumps vaccination: proof of one dose or laboratory immunity if born after 1956.
- Rubella vaccination: proof of one dose or laboratory proof of immunity if born after 1956.
- Hepatitis B vaccine: proof of three doses if enrolled in 12 or more credits.
- Meningococcal Meningitis vaccine type A,C,Y,W135: within the last five years; one dose on or after age 16. Required for all new students: age 18 and younger; age 19 and older and living in campus housing; and for certain medical conditions as outlined in the meningococcal questionnaire. MenB vaccine is only required for new students with certain medical conditions as outlined in the meningococcal questionnaire.
IMMUNIZATION RECOMMENDATIONS (NOT REQUIRED):
- Varicella (Chicken pox)
- Human Papillomavirus (HPV)
- Influenza
- Hepatitis A
- Pneumococcal (Pneumonia/for high risk groups)
- Tetanus vaccine/Tdap within the last 10 years
- Covid-19 vaccines per CDC recommendations
